ACCESS is a division of The Coordinating Center, providing technical assistance, consultation and training on the spectrum of topics associated with community care coordination of services for people with complex medical needs and disabilities. Fusing the disciplines of care management, quality improvement and nursing informatics, the goal of our services is to assist agencies, organizations and service providers with access to the most current thinking and best practices in the field of care management related to special health care needs.

Because ACCESS Group is an integral part of The Coordinating Center, staff members share the vision of The Center in working toward the full community inclusion of people with complex medical needs and disabilities and the overarching value of family and person centeredness. ACCESS Group staff members also have the available expertise of hands on care managers and service coordinators working in the field with individuals with the most challenging health care needs, using the most current methodologies and approaches.
